INSTRUCTIONS:
Step 1: Before submitting this electronic form, students must have a SRJC Student Identification Number (SID), and a current SRJC application for the term students plan to attend.
Step 2: A student's parent or guardian needs to complete the Parent/Guardian Consent Form to authorize the student's attendance at SRJC.
Step 3: Students and parents should review the Special Admit High School Dual Enrollment Expectations and Responsibilities
Step 4: Students should select their high school from the list below to begin the electronic High School Dual Enrollment Request Form.
Step 5: Once the form is submitted by the student, the principal or designee of the high school will receive an email for signature from Santa Rosa Junior College via Formstack. A principal or designee must also provide their signed approval on the form for the student to enroll.
Step 6: Once electronic signatures are collected from the principal the form will be routed automatically to Admissions & Records for processing. Students will receive an email from A&R once the form has been processed.
