Step 1: Before submitting this electronic form, students and parents should review the Special Admit High School Dual Enrollment Expectations and Responsibilities
Step 2: Students should select their high school from the list below to begin the electronic High School Dual Enrollment Request Form.
Step 3: Once the form is submitted by the student, the parent of the high school student will automatically receive an email for signature from Santa Rosa Junior College via Formstack. Parents must provide their signed approval on the form for the student to enroll.
Step 4: Once the form is signed by the parent, the principal or designee of the high school will receive an email for signature from Santa Rosa Junior College via Formstack. A principal or designee must also provide their signed approval on the form for the student to enroll.
Step 5: Once electronic signatures are collected from both the principal and parent, the form will be routed automatically to A&R for processing. Students will receive an email from A&R once the form has been processed.
